Heart Failure Core Measures: Genesis Medical Center Hospitals

Heart failure is a weakening of the heart’s pumping power. With heart failure your body doesn’t get enough oxygen and nutrients to meet its needs. These quality measures show some of the standards of care provided, if appropriate, to someone who has heart failure.  These tables and graphs include six months of data beyond what is available on the CMS Hospital Compare Web site.


With our continued commitment to being a premier cardiac facility, we have created a multi disciplinary approach to the identification, care, and discharge planning of heart failure patients.  Nursing units are doing real time chart audits to ensure not only is the appropriate care being given, but also that documentation reflects this.  Social workers, dieticians, cardiac rehabilitation, resource utilization RN’s and providers (physicians and mid-level practitioners) assist during the discharge planning process, We have held staff and community education sessions on the diagnosis of heart failure. We are continuing to evaluate the care processes and system, and initiate change as needed.
