Instructions for Completing my e-health

• Any time you have a question about using My e-health , you can click on the on the right side of the screen to review the instruction files. If you need further assistance please feel free to email us at webmaster@genesishealth.com. We will make every effort to respond to you within 72 hours.
• If you have a question related to your health or safety please call your physician or the 911 emergency services immediately. You can call 563-421-2000 or 800-383-2575 to speak to Genesis On Call for general health information. We also suggest that you review our Health Library pages for general health information.
• We are very interested in your comments and suggestions. If you feel that any part of My e-health is unclear or difficult to complete please email us so that we can update our instructions and improve the site. webmaster@genesishealth.com
• Once you begin the Health Risk Assessment, which is located within the Health Planner section, you must complete it to receive your health planner report. If you stop before completing it you will have to begin again the next time you enter the site.
• If you want to leave My e-health and return to another part of our website, simply click on the Genesis Health System logo in the upper left hand corner of the screen.
• If you would like to add records for your family members then go to the section called Member Maintenance. You can also change personal information and your password in this section.
