Acute Injury Care & Physician Case Management

Genesis Occupational Health is an acute care walk-in type clinic designed to deliver care to injured or ill employees when the need arises. Our clinics, located in Davenport and Moline, offer you convenient options no matter which side of the river your employees are working.

Scheduled appointments are preferred for the management of cumulative trauma type injuries. Typically a single physician will treat the employee through the entire case.

The following are examples of conditions for which we offer treatment:

We encourage you to call ahead for acute injury care or send the employee with a Service Request form. We provide these forms for your use. You can obtain a supply of them by contacting Amanda Hess at 563-324-0696 X 307.

When the employee reports to the clinic for injury care, authorization for services will be assessed, if they do not have one of the above forms we will contact the employer for clarification. With all injury care they are asked to complete an Intake form prior to seeing the physician. It asks the employee to provide an injury date, a description of how the injury occurred and current symptoms. The form asks them to locate their symptoms on a diagram of the body. In addition, they are asked about past evaluation and treatment for the condition, other medical history and social history.

During the employees visit at the clinic a Status Report is completed which documents the diagnosis, recommendation for activity level and treatment. A copy of this is provided to the employee with directions to return it to the employer for further directions on work activities. In/out time is also listed on the form. Scheduling times for follow-up visits are done by patient request unless we are aware of individual employer policies. We will follow employer policy for scheduling if you have a preference.

Nurse Case Coordinator of Referral Care & Therapy

At times certain cases will require advanced diagnostic testing or involvement of other specialists. Michelle Hostens currently serves in the role of case coordinator. This service is free to employers using our clinic. Michelle interacts with the physicians in the clinic and coordinates testing and referral for injured or ill employees. She provides valuable communication between employers, insurance carrier, testing or referral sites and the employee. She can be reached at 309-764-0684 ext. 0.
