Selecting a Skilled Nursing Facility

Selecting a Skilled Nursing Facility

MercyOne Genesis, in partnership with WesleyLife, is proud to offer The Summit, new to our community in 2022. Located in the heart of Bettendorf, The Summit offers independent living, assisted living memory care, short term rehabilitation and long term care, on the comforts of one campus. With state-of-the-art facilites and amenities, we encourage you to seek out The Summit for all of your residential and health care needs.

In addition to The Summit, MercyOne Genesis offers both short term rehabilitation and long term care services at Silvis (Illini Restorative Care), and DeWitt (Fieldstone of Dewitt). Each of these facilities is highly regarded for their service, safety and quality of care. Should these locations be most appropriate for you, we welcome the opportunity to seamlessly continue your health care services within one of our outstanding facilities.

We recognize that these locations may not work for all of our patients and their families. We have engaged in partnerships with area facilities, placing full time MercyOne Genesis Nurse Practitioners in these facilities to enhance the level of care and medical oversight. Nurse Practitioners currently provide services at the following MercyOne Genesis Preferred Providers:

Please note that the likelihood of a patient requiring readmission to the hospital drops by 50% when a patient goes to a facility where a MercyOne Genesis Nurse Practitioner is on site five days/week.

The decision to admit to a skilled nursing facility is often a difficult and emotional decision for the patient and family. This is more challenging in light of the recent COVID-19 pandemic. We strongly encourage those patients and families making these decisions to seek services at the locations noted above.

Medicare provides a five-star rating program for many health care organizations, including skilled nursing facilities. You can review the most current findings at www.medicare.gov/nursinghomecompare/search.html

You may find facilities in the area with similar five star ratings. Some facilities achieve higher ratings, but do not provide care for acutely-ill patients. This is true in the Quad Cities. Our Wellspire Communities (The Summit, Fieldstone of Dewitt, and Illini Restorative Care) have established themselves as consistent providers of quality care over many years. Other pertinent questions to ask of facilities when considering admission include:

  • What is your current rate of readmitting patients back to the hospital within 30 days?
  • How frequently do you care for people with my (or my family member’s) medical condition?
  • How many days per week can I (or my family member) expect to receive physical, occupational and speech therapy services at the facility?
  • How often will a physician or nurse practitioner be available to see me (or my family member)?

Each patient and family’s needs are different, and identifying the correct fit for you includes more than just the closest facility, or the one with the newest décor. We hope this information is helpful in your decision-making process. Should you have further questions, please contact your Care Coordinator.
