Family Practice/Primary Provider
- Sees you for routine and preventative care visits, and assists you in managing your chronic conditions
- Refers you to the program
- Collaborates with other health care team members to manage your care
Specialist Provider
A specialist provider is a provider, other than your primary care provider or family doctor, who specializes in a specific area of medicine. Examples could include Endocrinologist, Cardiologist, Rheumatologist, etc.

Pharmacist
- Performs comprehensive review of medications
- Assesses and advises on best medication therapy selection and management based on person specific conditions, insurance coverage, and costs
- Educates on the purpose and interactions of each medication in relation to your health
Nurse Care Coordinator
- Reviews clinical data as available
- Provides ongoing comprehensive individual assessment, education, support, and referral to other care management team members and services as needed
Social Worker
- Assesses medical condition, social and emotional needs and determine how they may impact your health
- Provides mental health counseling as needed
- Identifies and connect you to community and social support systems as needed
Dietitian
- Develops meal and nutrition plans based on patient preference, budget, and special dietary needs
- Promotes healthy food choices through education
Medical Assistant
- Coordinates registration and scheduling appointments
- Ensures insurance coverage and billing are reviewed
- Assists with patient monitoring and clinical data collection
