Your First Visit
- Arrive 30 minutes prior to your appointment.
- All patients and visitors are required to wear a mask at all times.
- Please bring the following items:
- Picture ID
- Your insurance cards
- A list of all medications, vitamins, or herbal supplements you normally take, or their original containers
- A list of your allergies
- A copy of your advance health care directive
- A list of questions for your doctor
- Completed new patient paperwork
