As a courtesy, MercyOne Genesis Medical Group will submit claims to your primary and secondary insurance. Patient co-pays and deductibles are due at the time of service. If MercyOne Genesis Medical Group does not participate with your insurance, payment in full is required at the time of service.

Patient statements are sent monthly and payment is due upon receipt of your first statement.Services denied by insurance as non-covered charges will become your responsibility. If your balance cannot be paid in full, payment arrangements can be made by contacting our billing office. Office hours are 8 a.m. to 5 p.m. Monday through Friday. For your convenience, payments for all MercyOne Genesis Medical Group providers can be made at any location.

Copays/Deductible and Co-Insurance

All copays are due at the time of service. This arrangement is part of your contract with your insurance company. A minimum payment of $20.00 is due for insurance plans with deductibles and co-insurance.

Failure on our part to collect copayments, deductibles and co-insurance from patients can be considered fraud. Please help us by paying your copayment at each visit. You may be asked to reschedule if payment is not made at the time of service.

Insurance Claims

Insurance coverage will vary according to the type of plan you have. It is recommended that you familiarize yourself with the terms of your insurance coverage. It is your responsibility to know which practitioners/ services are covered by your plan and which services are your responsibility to pay. GHG is not responsible for knowing your specific insurance plan coverage and limitations. To ensure your services are billed correctly, please present your current insurance card(s) at each office visit.

If you have an HMO or PPO plan, you may have special requirements such as pre-certification for certain tests or procedures. You are responsible for knowing your plan’s requirements and making sure they are met. If requirements are not followed, insurance payments may be reduced or denied, thus increasing your patient due amount.

If you do not have an up to date insurance card, payment in full for each visit is required until we can verify your coverage. If you fail to provide us with correct insurance information in a timely manner, you will be responsible for the balance.

Obstetric Patients

Financial obligation for obstetrical care will be determined at the first visit and a contract will be completed and signed for patient responsible amounts. Labs, and any office visits unrelated to pregnancy, are not included in your global contract and will be billed separately. Balances for which the patient is responsible must be paid prior to delivery as designated in the contract. The prepaid OB balance will be applied to the charge following delivery. After the insurance payment is made, any remaining credit will be applied to all outstanding MercyOne Genesis Medical Group patient balances. If a credit remains after all patient-due balances have been paid, the patient will receive a refund. Credits will not be held for any pending insurance claims unrelated to the delivery.

Annual Examinations

Insurance coverage for annual exams and other wellness services varies greatly among insurance plans. Some services may not be covered and your insurance company may not consider others medically necessary. Your practitioner may recommend non-covered services in the best interest of your health. We recommend that you contact your insurance company to verify coverage before scheduling annual or wellness exams. MercyOne Genesis is not responsible for knowing your specific insurance plan coverage and limitations.

It is difficult to quote an exact price for services provided because all patients receive individualized care tailored to their needs. Your practitioner’s office may quote a price range with the understanding that the price is only an estimate and may vary according to your practitioner’s evaluation and services provided.
