Avoid paperwork and waiting by pre-registering online. All you'll need is 15 minutes and the following information for the patient: physician name, insurance information, occupation, place of employment and employer's phone number.
Once the form is submitted, you'll receive an email that your registration has been completed. The email will include a link to a secure Web site where you can check registration status and ask follow-up questions.
